How to Use This Estimator
1. Start with Part B. Keep $209.50 only as the Trustees projection, or replace it with the figure on your notice. Add any IRMAA surcharge you actually pay.
2. Enter real plan prices. For Medicare Advantage, use the premium and MOOP from the plan Annual Notice of Change mailed by September 30. For Medigap and Part D, use the quotes for your ZIP code.
3. Estimate a normal year honestly. Count last year doctor and specialist visits, hospital stays, imaging, therapy, and drug spending at plan prices. Then look at the worst-case column before you decide.
4. Verify before enrolling. Check every doctor, hospital, and prescription at medicare.gov/plan-compare. This estimator does not check networks or formularies.
When Each Path Tends to Win
Medicare Advantage tends to win the expected-total column in a light-use year because the extra premium can be $0 or low. It loses predictability: copays rise with use, networks and prior authorization apply, and the worst case runs to the plan MOOP.
Original Medicare plus Medigap tends to win the worst-case column. You pay Medigap and Part D premiums every month, but a bad health year adds little beyond the Part B deductible on many popular Medigap plans. It also keeps nationwide access to providers who accept Medicare.
Original Medicare plus Part D with no Medigap can look cheap in a healthy year and turn expensive without warning. There is no cap on the 20 percent Part B coinsurance. Treat Path C as the exposure warning, not the safe default.
